Q: What is Functional Medicine?

Q: What sort of conditions does Foundations treat?

A: Dr. Comfort uses functional medicine to treat many chronic conditions, and also offers personalized wellness for optimal health. If you have more questions about a specific condition, please send questions to info@foundmed.org

Q: How long will it take me to reach my health goals?

A: While some clients may see improvements in as little as a few weeks, treatment time varies with each individual's needs. Care at Foundations begins with proper identification of underlying causes, which takes time.

Q: Will I have to stop seeing my other health care providers?

A: As Foundations' focus is on functional medicine, it is not able to provide primary, routine, specialty, acute or urgent care services, and you should receive care in these areas as needed. Continuing your care also allows your health care team the opportunity to adjust any relevant treatment plans as you improve.

Appointments

Q: How do appointments with Foundations work?

A: Foundations provides you with the personal service that doctors used to provide long ago. Dr. Comfort comes to you, so appointments are in the comfort and convenience of your own home. Video appointments are also available if preferred.

Q: How do I make an appointment with Foundations?

A: You can request appointments with Dr. Comfort here, via the client portal.

Once you register as a new client, you will receive emails confirming your request, and information on how to fill out pre-appointment paperwork.

Q: Why must I complete my questionnaires up to a week prior to the appointment?

A: Providing a detailed history allows for a better assessment, and an in-depth review of this information takes time. You can sign into the information-gathering app as many times as needed to complete each of the forms.

 

Q: Why does Foundations come to me?

A: Being seen at home opens the door to a deeper understanding of your environment, lifestyle, the tools available to you, and how best to integrate treatment into your daily life. Video appointments are also available if preferred.

 

Q: Which locations does Foundations serve?

A: To see if services are available in your area, please e-mail Foundations at info@foundmed.org. Appointments are also available virtually.

 

Q: How can I access my appointment summary and other health information?

A: Foundations uses a secure electronic medical record to store information, which you can access through the client portal.

 

Service Charges and Insurance

Q: Does Foundations take health insurance?


A: Foundations accepts payment directly from clients. If clients choose to submit costs to health insurance plans, Foundations can provide forms to assist your insurance provider in assessing any reimbursement. The service cost reflects time with the physician, as well as any research time required in the development of a treatment plan - personalized for you.

Q: What forms of payment does Foundations accept?

A: Foundations accepts cash, credit or check as payment, prior to or at the time of service. Foundations also works with clients who participate in cost-sharing programs  to pay for their health care. You may also use HSA or FSA funds. Using cash or check avoids the service fee charged for credit or debit card payments. If clients choose to submit costs to health insurance plans, Foundations can provide forms to assist your insurance provider in assessing any reimbursement.

Q: Is laboratory testing covered in the cost of the services?

A: Laboratory tests and recommended treatments are not included in the cost of services at Foundations. The service costs reflect time with the physician, as well as any research time required in the development of your personalized treatment plan. Some laboratory tests can be covered by health insurance, and in addition, HSA and FSA funds are often accepted for lab tests and treatments.
